Patient Information

Please bring the following paperwork to your appointment:

Your patient information form (download the .pdf or submit online), as well as:
1. Medical Information: Please bring copies of any letters or medical records from other health care providers. Medical records for the past 5 years are necessary to file for insurance and to give the doctors a complete medical history.
2. Physician Recommendation: If filing for insurance, please provide a written recommendation for weight-loss surgery from your primary care physician.
3. Diet / Weight-Loss Info: Any documentation proving you participated in any diet / weight loss attempts. (This includes office notes, receipts or prescriptions.) Exercise and Nutrition form: Download the .pdf and bring it with you or submit online.
4. Insurance card
5. Driver's License
6. Physician Referral: If your insurance plan requires a referral for this consultation, please contact your primary care physician and have one faxed to our office fax at (717) 627-6921 before your appointment.

Our Program

Step 1: Seminars

Attend Free Seminar

The first step toward long-term weight loss is to attend one of our seminars to learn more about your weight loss surgery options and the BMI of Lancaster program.

Register for an upcoming seminar by calling (717) 627-0398. Private consultations are also available.

Step 2: Insurance

Call Your Insurance Company
Check to make sure weight loss surgery is covered by your plan. Criteria for coverage will vary among insurance companies.

Step 3: See the Surgeon

See the Surgeon
After attending a seminar and deciding that this is something you would like to pursue, please call the office to schedule an appointment with the surgeon.

Step 4: Consultations

To meet established guidelines for weight loss surgery, consultations with a registered dietician, an exercise physiologist and a behavioral health specialist are required.

Step 5: Pre-operative Visit

You are moving forward with surgery and to prepare you for your new beginning, will need to attend the following appointments:
